The statute caps what the form may ask — names, contacts, birth date, a simple description of the education being provided, and your signature. There are no subject, hour, testing, or record requirements anywhere in the law.
Exactly what it says — a sentence or two. The statute’s only qualitative test is that a program not be operated to dodge the compulsory-attendance law; it authorizes no curriculum review. Families typically write something like “individualized instruction in core academic subjects.”
